This form is included in the “Welcome Packet” document. However, please feel free to download, complete, and return it to the office, so we may release and/or obtain confidential information.

This form is to be filled out before the first visit if the patient is a minor and it must be filled out by the patient’s teacher. This form may also be completed if you are instructed by your clinician to do so; however, please keep in mind that it must be completed before their next visit.
